It's crucial to bear in mind that any envelope containing Form I-693 that has been opened or tampered with will be rejected by USCIS. 

Form I-693, Report of Immigration Medical Exam and Immunization Documentation

This necessary form, recently updated, must be filled out by a USCIS civil surgeon. It plays a vital role in the process of applying for Green Card or lawful permanent resident status through adjustment of status.

Where to Submit Form I-693 

Following the medical evaluation, Prospect Park Medical will furnish the completed Form I-693 in a sealed envelope for submission to USCIS. The applicant must not alter this envelope. Submission instructions vary depending on the individual's application status and are outlined by USCIS. 

 

Special Instructions USCIS mandates that medical evaluations for Green Card applicants conducted after January 1, 2012, must employ the current version of Form I-693. Older versions will be turned down, necessitating the applicant to revisit the civil surgeon for resubmission.
